• Community Nurse, Virtual Wards, Stoke on Trent
• £23 - £30 per hour
• Temporary Block Booking OR Adhoc
• Monday to Sunday – 08:00 – 20:00

Key Responsibilities:

• Experience with emis, undertaking assessments, providing holistic care to elderly patients on DN caseload
• Management of long term conditions and palliative care
• Aware of safeguarding adults
• Search for health needs and working with the core and wider MDT to meet patient needs

Qualifications:
- Registered Nurse (RN) with NMC registration.
- Continence care specialization preferred.
- Strong clinical, communication, and empathy skills.
- Previous similar role experience a plus 

- Essential Physical Health Assessment Qualification at level 6 or 7
